First detection of SAT1 FMD serotype in China

China is scrambling to contain its first domestic outbreak of the Southern African Territories Type 1 (SAT1) foot-and-mouth disease (FMD) strain, fast-tracking emergency vaccines as the agricultural sector faces a widening threat to its livestock industry.

According to Chinese media reports cited by Caixin Global, the SAT1 strain has been officially confirmed in the western regions of Xinjiang and Gansu. Existing national vaccines, which primarily target the traditional O and A serotypes, fail to provide cross-immunity against this strain. The virulence of the strain affects cattle, sheep, and pigs, with mortality in young cattle reported at up to 50%.

SAT1, along with SAT2 and SAT3, is historically endemic to sub-Saharan Africa, explains Prof Francois Maree of the Centre for Viral Zoonosis at the Department of Medical Virology, University of Pretoria. However, SAT1 is increasingly spreading beyond this region. The informal movement of animals is considered the most likely driver of this expansion, typically occurring in the absence of adequate veterinary sanitary controls.

Confirmed cases and vaccine shortcomings

According to a report by the Biothreats Emergence, Analysis and Communications Network (Beacon) run by the Boston University, China’s Ministry of Agriculture confirmed 219 cases of FMD in cattle across two establishments in the Gansu Province and Xinjiang Uyghur autonomous region.

The two affected establishments house a total of 6 229 cattle, indicating potential exposure of a substantial livestock population.

Current domestic vaccines in China do not offer cross-protection against the SAT1 serotype. In response, local authorities in Xinjiang and Gansu have implemented culling and disinfection measures to contain the outbreaks.

The emergence of the SAT1 serotype represents a significant shift in China’s FMD epidemiology. Historically, FMD in the country has been dominated by serotypes O and A. The lack of vaccine cross-protection creates a substantial immunological gap across China’s cattle population, markedly increasing vulnerability to widespread transmission.

This development underscores the urgency of developing or importing SAT1-specific vaccines to prevent further geographic spread.

Likely introduction routes and regional risk

Detection in Gansu and Xinjiang – both pastoral regions with high livestock density and established FMD endemicity – suggests the virus entered through livestock trade routes or cross-border animal movement. These northwestern provinces serve as critical livestock corridors linking China to Central Asian countries, creating pathways for the introduction of novel serotypes and potential onward transmission to other major cattle-producing provinces.

The United Nations Food and Agriculture Organisation’s (FAO) rapid risk assessment covering 2-23 February 2026 (published 04 Mar 2026) notes that SAT1, originating in sub-Saharan Africa, has demonstrated atypical geographic expansion since its first detection in Iraq in March 2025. Subsequent spread has been reported in Azerbaijan, Egypt, Bahrain (quarantine station), Cyprus, Kuwait, Greece, Iran, Israel, Lebanon, and Turkey, demonstrating significant transboundary transmission potential driven largely by informal animal movements.

FAO projections indicate that further spread of SAT1 is very likely in the coming months, particularly via small ruminants, which may be subclinically infected yet capable of shedding the virus.

Implications for FMD control in China

In recent years, FMD in China has become more stable, with only seven outbreaks reported between 2021 and 2023, most linked to animal movement. Serotypes O and A remain present, although serotype A has not been detected since 2019 and Asia1 since 2009. Current circulation is dominated by introduced strains such as O/Ind-2001 (primarily in cattle) and O/CATHAY (in pigs), with O/Mya-98 persisting at low levels. Despite ongoing viral evolution, existing vaccines have remained largely effective.

Control strategies rely on nationwide surveillance, compulsory vaccination, and international cooperation, which have maintained a generally stable epidemiological situation.

Therefore, the introduction of SAT1 represents a qualitative departure from historical patterns dominated by serotypes O and A.

The trajectory of this outbreak will depend critically on three factors: the speed of SAT1-specific vaccine development and deployment, the effectiveness of movement restrictions in preventing inter-provincial spread, and the extent of undetected transmission prior to confirmation. Warning signals of escalation would include detection of SAT1 cases beyond Gansu and Xinjiang, particularly in major livestock-producing provinces such as Sichuan or Heilongjiang, evidence of aerosol transmission beyond the initial establishments, and serological surveys indicating widespread subclinical infection, suggesting delayed recognition of the outbreak. – Izak Hofmeyr, Plaas Media
